New Patient Registration

Patients are required to sign their registration forms.  If you do not have access to a printer you can come in to the surgery to complete and sign your form and show your photographic and proof of address ID.

You are also required to complete additional forms as part of the registration process and these forms can be printed off from the link below, completed and returned to the practice with the required documents  - New Patient Registration Pack

Patients can register as a new patient at Cove Bay or Kincorth Medical Centre.  Once you are registered with the practice you can be seen at either site.

In addition, you will be invited to attend a registration appointment with one of our Healthcare Assistants, allowing us to provide medical care in the interim period whilst your medical records are transferred from your previous practice.

If you move out of our practice area, you will need to register with a new practice as soon as you move. We can provide information about how to find a new practice.
